Preferred Provider for Tricare, Blue Cross/Blue Shield, and Premera. If you owe a copayment, it will be collected on the day of your session. Other arrangements can be made if needed
Please note that I can blll any insurance whether I am a preferred provider or not.
Please note that all co-payments (amount not covered by your insurance company) are due at each session.
As a courtesy to you, I will bill your insurance company through my billing company Goldenmountain Group Inc. They are based in Washington State and owned and operated by Tami Honsaker. She can be reached easily by email at firstname.lastname@example.org she is always quick to respond to questions and inquires and if something more complicated arises, she runs it by me. Payment plans can be made if necessary. It is my philosophy that financial situations should never get in the way of getting help, so please just speak with me and we can work out a variety of arrangements to get you help.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by callling your insurance carrier prior to your first visit and asking the following questions, if you need help with this, my billing company is happy to assist.

*What is my deductible and has it been met?
*How many sessions per calendar year does my plan cover?
*Is approval required from my primary care physician?
~Reduced fee services are available on a case by case basis for fee for service clients to be determined by the therapist.
Self Pay Clients
~Clients who wish to pay out of pocket for their counseling are most welcome. Please inquire about the fee for service option.
~Cash, check, money order, and all major credit cards accepted for payment.
~Appointment times are very important to keep, both for your treatment and others. I work hard to keep appointments starting and finishing on time to be courteous to you and the next client. Of course emergencies arise but because our time is valuable, I have a firm cancellation and no show policy (barring emergencies). If you do not show up for your scheduled appointment, or cancel with less than 24 hours notification (via email or phone), you will be required to pay a $100.00 fee at the beginning of your next scheduled appointment. This fee is not billable to insurance companies so therefore, it is your responsibility.
